Enrollment Meeting Eligibility Form

Step 1 of 13
Step 1

Ready to Enroll?

If you’re ready to schedule your final enrollment meeting with FLT Academy, please complete this brief form to ensure everything is ready to go.

Step 2

Have you obtained a First Class Medical Certificate?

We cannot schedule your Final Enrollment Meeting until you have been issued a valid FAA First Class Medical Certificate.
Step 3

Do you have a valid U.S. driver’s license?

A valid driver’s license is required for enrollment.
Step 4

Are the name and address an exact match on your driver’s license and FAA First Class Medical Certificate?

The FAA requires an exact name and address match on your driver’s license and medical. This does not make you ineligible for enrollment. However, you must update one or both so they match before you will be eligible to take any FAA tests, including written exams, practical exams, or in-house evaluations/EOCs.
Step 5

Are you planning on paying out of pocket for training, or are you financing?

Step 6

Have you finalized your loan pre-approval?

Have you received an email from your lender stating, “your loan is at your school for certification”?

If you haven’t received this notice, that means there are still steps remaining to complete your loan pre-approval. Please contact your lender to see what you still have left to do to complete this step. Your loan must be ready for certification before you are eligible to schedule your final enrollment meeting.
Step 7

Are you a U.S. citizen or permanent resident of the United States?

Have you completed a profile with the TSA Flight Training Security Program and been approved for flight training?
If you are not a U.S. citizen or permanent resident and have not obtained TSA approval for flight training, you must do so prior to enrollment.

Open the TSA FTSP website
Step 8

Do you have your proof of citizenship? Does the name listed on your passport and/or birth certificate match the name on your medical and driver’s license?

Your proof of citizenship must match your ID and medical. If you have changed your name through marriage or another legal process, you must also provide documentation regarding that change, such as a marriage certificate or petition for change of name.

Do you have documentation showing the name change?

Please complete that step, then return to schedule your Final Enrollment Meeting.
Step 9

Have you completed your drug screening?

You must complete your drug screening before scheduling your Final Enrollment Meeting.
Step 10

Have you obtained your background check?
